AmaZulu are out of the Nedbank Cup following a 3-2 loss on penalties to PSL top-flight side Jomo Cosmos on on Tuesday night.
The match was decided from the penalty spot after 120 minutes of football produced a 1-1 draw in Potchefstroom.
Former Bidvest Wits player Denver Mukamba gave Cosmos the lead midway through the first half and Steve Barker's men levelled in the 72nd minute through Zambian midfielder Allan Mukuka.
Usuthu suffered a big blow when captain on the night Marc van Heerden had to go off with an injury just before the half tie break. Tshepo Liphoko took his place, but the loss of AmaZulu's left footed dead ball specialist made an impact.
AmaZulu were then dealt another blow in extra time when lanky striker Mame Niang was sent off for dissent after appealing for a penalty in the 106th minute.
Cosmos held their nerve during penalties and march into the Last 16 round. They will find out their next opponents on Thursday when the PSL holds the last-16 draw at the Carlton Centre in Johannesburg.
